What Information Do I Need to File Bankruptcy?

Once you have retained me to represent you, you will be provided a bankruptcy questionnaire. Fill out the questionnaire, then call to schedule an appointment. I will go over the questionnaire to analyze your financial circumstances.

You will need to provide me with the following materials:

  • Tax returns for the last four years with W-2s;
  • Bills (and credit report, if available) for all debts;
  • Pay stubs for 6 months and proof of all money received for 6 months;
  • Bank Statements for 1 month;
  • Tax assessor’s statement for real property that you own;
  • Taxes owed;
  • List everything that you own and its value and
  • Business financial statements, if you own a business;


You must make sure that you are reporting everything that you own and all that you owe. You must sign the petition under penalty of perjury which carries a 5 year prison sentence and a $500,000 penalty for violation. It is very important that you be thorough and accurate.
